Patient of the Month

YOU Could Be Our Next Patient of the Month and Win Cool Prizes!!

Would you like to be Patient of the Month? Once a month Dr. Chung draws one name from our raffle of OUTSTANDING patients. In order to be entered into the drawing, you have to earn a raffle ticket by following these rules:

1. Don't miss or come late to your appointment
2. No loose, broken, or missing brackets, bands, wires, appliances
3. Have exceptionally clean teeth and gums
4. Wear elastics as instructed
Patients of the Month will receive one of a variety of prizes. Maybe you can be next month's winner!

Patient Referral Rewards

The highest compliment you can give our practice is to share your experience with others by referring them to our office for their orthodontic treatment. To thank you for this courtesy, we would like to return the favor to those patients who invite their friends to join our practice. Just refer a friend and have that person mention that you sent them at their exam. Once that person begins active treatment we would like to present you with a gift of appreciation.

Rules and Guidelines:
- Referral must start recommended treatment
- Outside of immediate family
- New referrals only
- Must bring in the referral form signed from the patient
- Referral must be a current or former patient

Plus, you will be entered into our raffle to win a Grand Prize every 6 months!!

Monthly Contest

When you come in for our regular visits, you get to participate in our monthly contests. Here are some examples of our monthly contests.

	What Are Your Plans This Summer?
	Tell us what you'll be doing during your summer vacation. Three winners will be drawn to receive gift certificates toward vacation shopping supplies at the mall.
	Where In the World?
	Identify famous landmarks from around the world for your chance to win an iTunes Gift Card, compliments of Dr. Chung. Winners will be drawn from entries with the most correct answers.
	National Youth Art Month
	March is national Youth Art Month. Submit your drawing to be entered to win a gift certificate to Michael's.
	Match the Presidents to the Money
	In honor of President's Day match the U.S. President with the dollar bill he is printed on. If you get them all right you could win a Visa gift card!

	National Book Month
	January is National Book Month. Tell us your favorite book and be entered to win a gift card to Barnes and Noble.
